The way a foam mattress works is to have a foundation support the mattress, an interior 'stiffener' to support the memory foam, and the memory foam to support your body. You can see a good visual of this at  http://www.kaixiangcn.com/product/53.html

Adding a memory foam mattress pad (not a topper) does make sense. This reduces the friction wear on the foam top layer, prevents soiling, and helps the mattress last longer. These are the same fitted mattress pads you'd use on a regular mattress. Just make sure the pockets (corners) are deep enough for the mattress since most foam mattresses are deeper than standard mattresses
